Position Summary:

The Licensed Vocational Nurse (LVN) Provides care of people with developmental and/or physical disabilities who require 24 hour consistent care. LVN provides complex vocational nursing work, that involves providing for the care, treatment, and general welfare of individuals. Works under the direction and supervision of the Operations Manager and Administration Manager while performing nursing duties within the assigned areas and assigned shift. Keeps informed of and follows all agency and facility policies and procedures.

Responsibilities:

· Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime

· This position is a mandated reporter, and is expected to observe all consumers rights, report unusual occurrences and suspected dependent adult abuse, as specified in Title 22, California Code of Regulations.

· Practices nursing care within the scope of the CA Board of Vocational Nursing and Psychiatric Technicians.

· Maintains confidentiality and compliance with HIPAA.

· Performs and/or directs nursing procedures, such as administering medications and treatments including oral medication, hypodermic injections, urinary catheterization, enemas; taking and recording temperature, pulse, blood pressure, respiration and other health care procedures authorized within the scope of practice for the Vocational Nurse license and according to physicians’ orders and instructions.

· Maintains the health of persons served by conducting physical assessments, monitoring health and dietary needs.

· Provides supports to ensure all basic needs, i.e., grooming, hygiene, provision of hydration and nutrition, medication assistance are met in a timely, skilled and thorough manner.

· Seeks an RN to intervene where there is need for monitoring or administrating medication or medical plans.

· Initiates CPR and first aid as needed.

· Assures the delivery of health-related services and provides nursing services to maintain health, maximize independence and support.

Minimum Qualifications (Experience and Skills):

· Proof of current license as an LVN in good standing in the state of California.

· One year experience in the health, human services and/or community services field preferred.

· Experience working with people who have mental health/intellectual developmental disabilities preferred.

· Ability to maintain current CPR certification.

· Ability to work in a home-like setting.

· Knowledge of individual service plans, including understanding of client rights and health needs

· Knowledge of nursing procedures and techniques and the principles and process used in providing patient care.

· Ability to provide patient care and to prepare and maintain records, to communicate effectively.

· Ability to apply principles and practices to assess, prevent and control diseases.

· Must have strength and flexibility to safely perform Cardiopulmonary Resuscitation (CPR).

· Must be able to demonstrate a working knowledge of emergency procedures and emergency equipment.

· Ability to clear Department of Justice (DOJ)/Federal Bureau of Investigation (FBI) fingerprint and criminal record clearance.

· Ability to obtain or in possession of a current health screening and TB test as a condition of employment.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; sit; bend; walk; and reach with hands and arms. This position requires the ability to occasionally lift up to 50 pounds.
