Responsibilities & Requirements

The Manager Care Management I leads and coordinates care management services to support patient-centered care, safe transitions, and optimal health outcomes. This role ensures regulatory compliance, resource allocation, and effective interdisciplinary collaboration to deliver high-quality, efficient care management programs. Performs other duties as assigned. All job responsibilities are to be carried out in alignment with Kettering Health's mission, vision, values, and B Safe program. Bachelor's degree in Nursing (BSN) or Master's degree in Social Work Minimum of three years of experience in utilization management, case management, discharge planning, or related care coordination roles Current and valid Registered Nurse (RN) or Licensed Social Worker (LISW-S) license in the State of Ohio obtained within 3 years of hire

Preferred Qualifications

Master's degree in Nursing, Healthcare Administration, or related discipline Supervisory or team lead experience in care management or social work Certified Case Manager (CCM) or equivalent professional certification
