POSITION PURPOSE
The Manager, Health leads the clinical and community health portfolio of the Yuułuʔiłʔatḥ Government Health & Social Services department. Reporting to the Director, this role is accountable for the safe, culturally grounded delivery of home and community care, primary health support, chronic disease management, health promotion, immunization, patient travel, and partnerships with the First Nations Health Authority (FNHA), Island Health, and West Coast General Hospital (Huupatu). The Manager supervises the nursing team, health care assistants, homemakers, patient transporter, and Program Lead, Health Services, and ensures programs respond to community needs while meeting professional standards, funding agreements, and YG policy. The Manager works in close partnership with the Manager, Social Services to deliver integrated, wraparound wellness services to YG citizens across hitac̓u, Ucluelet, and Port Alberni.
ACCOUNTABILITY
Health Program Leadership
- Oversee the planning, delivery, and evaluation of community and home care, primary care support, chronic disease management, palliative care, immunization, and health promotion programs.
- Ensure services are culturally safe, trauma-informed, and grounded in Nuu-chah-nulth values and Yuułuʔiłʔatḥ priorities.
- Translate the Director's strategic direction into operational workplans, service standards, and measurable outcomes.
- Monitor program quality, identify service gaps, and recommend improvements based on community feedback and health data.
Clinical Governance & Safety
- Ensure nursing and allied health services are delivered within scope of practice and in line with BCCNM, FNHA, and Island Health standards.
- Oversee clinical protocols, care planning processes, medication management, infection prevention, and documentation practices.
- Support the nursing team with clinical oversight of HCAs and Homemakers.
- Escalate serious clinical incidents to the Director and support review, learning, and corrective action.
Team Leadership & People Management
- Directly supervise the Program Lead, Health Services, Community Health Nurse, LPN, HCAs, Homemakers, and Patient Transporter/Driver.
- Recruit, onboard, coach, and develop staff in partnership with the Manager of Human Resources.
- Set clear expectations, complete performance reviews, and support ongoing learning, including cultural competency and clinical development.
- Foster a healthy, respectful, and psychologically safe team environment, and support staff wellness in emotionally demanding work.
Patient Travel & Access to Care
- Oversee patient travel services of citizens of hitac̓u to their out-of-town medical appointments, including overnight travel arrangements where needed.
- Ensure trip logs, mileage, and reimbursements are maintained in line with the FNHA Medical Transportation program.
- Remove access barriers for citizens without a driver's licence or reliable transportation, particularly Elders and clients with mobility needs.
Partnerships & Systems Navigation
- Maintain strong working relationships with FNHA, Island Health, physicians, specialists, public health, and community health partners.
- Represent YG Health at inter-agency tables, case conferences, and regional health planning as directed by the Director.
- Support citizens and families in navigating provincial and federal health systems, and advocate for culturally safe care.
Financial & Funding Accountability
- In partnership with the Director and CFO, manage the health portfolio's budget, funding agreements, and contribution reporting.
- Contribute to funding proposals for health programs and initiatives.
- Monitor spending, approve expenditures within delegated authority, and ensure value for money.
Administration, Reporting & Compliance
- Ensure timely, accurate submission of funder and internal reports.
- Maintain confidential health records and administrative systems in line with PIPA, YG policy, and health information standards.
- Support the Director in briefing materials for the CAO, Executive, and Legislature as required.
- Ensure compliance with lone-worker, WorkSafeBC, and emergency response protocols.
Integrated Wellness & Community Engagement
- Partner with the Manager, Social Services to deliver wraparound supports that connect health, social, cultural, and justice services for citizens and families.
- Participate in community events, wellness initiatives, and cultural gatherings.
- Engage citizens, Elders, and families in the design and continuous improvement of health services.
EDUCATION & EXPERIENCE
Required:
- Bachelor's degree in Nursing, Health Administration, Public Health, or a related health/social discipline (equivalent combination of education and directly related management experience will be considered).
- Minimum 5 years of progressively responsible experience in community health, primary care, or home and community care, including at least 2 years supervising clinical or health services staff.
- Working knowledge of the BC health system, First Nations Health Authority, Island Health, and FNHA Medical Transportation.
- Demonstrated experience leading culturally safe, trauma-informed services in an Indigenous or community-based setting.
- Strong operational, budget, and reporting skills, including experience with funding agreements and contribution reporting.
- Valid BC Class 5 Driver's Licence and access to reliable transportation.
- Satisfactory Criminal Record Check, including Vulnerable Sector screening.
Preferred:
- Active practicing registration as an RN with the BC College of Nurses and Midwives (BCCNM).
- Experience working with First Nations governments and understanding of self-determination in health.
- Familiarity with Nuu-chah-nulth culture, language, and West Coast geography.
- Training in trauma-informed practice, mental health first aid, and cultural safety (San'yas or equivalent).

WORKING CONDITIONS
- Primary place of work is the Cixʷatin Centre and health facilities in hitac̓u, with regular travel to Ucluelet and Port Alberni.
- Combination of office-based leadership work and time in clinics, community settings, and homes with the team.
- Occasional evening or weekend work for community events, emergencies, or leadership responsibilities.
- Exposure to sensitive or complex health situations; supports staff working in emotionally demanding roles.
- On-call or after-hours availability may be required for urgent health or staffing matters.
