Role Overview
We are seeking a Manager of Clinical Practice (MCP) to provide strong clinical leadership, operational oversight, and excellence in client care within a community care setting.

The MCP promotes full-scope clinical practice, ensures compliance with professional and regulatory standards, and drives quality improvement initiatives. This pivotal role provides clinical oversight, team leadership, and dedicated mentorship across both professional nursing staff and paraprofessional care teams – guiding and supporting staff growth to ensure optimal outcomes for the clients we serve.

Key Responsibilities

  • Oversee branch clinical quality standards by conducting regular chart reviews, identifying opportunities for operational improvement, and driving clinical quality initiatives.
  • Ensure comprehensive client assessments are completed and oversee the development of clear, client-centered care plans that align with professional practice standards.
  • Conduct client home visits as needed to evaluate care needs, monitor service delivery, and ensure optimal matching between clients and field staff.
  • Act as a reliable point of contact to investigate, document, and resolve client, family, funder, or employee concerns in a professional and timely manner.
  • Supervise, coach, and evaluate field staff, providing strong clinical leadership and mentorship to paraprofessionals to foster their professional growth and ensure high-quality client care.
  • Lead practical orientation, clinical training sessions, and routine performance management to maintain team competency and support ongoing professional development.
  • Coordinate care delivery through clear, professional written and verbal communication with community funders, interdisciplinary care teams, and government health programs.
  • Support overall branch operations and participate in a structured on-call clinician rotation as required to ensure continuous clinical support for clients and staff.
  • Maintain strict compliance with CNO professional standards, Bayshore policies, occupational health and safety regulations, and privacy legislation.
  • Other duties as assigned from time to time.
