Summary/Objective
The Quality Improvement Manager serves as the primary quality contact and subject matter expert for the providers, leaders, and staff of Cameron Medical Group. This position is responsible for the overall administration, monitoring, and improvement of clinical quality metrics and population health initiatives.
The Quality Improvement Manager provides providers and staff with timely, accurate, and actionable information, along with appropriate education and workflow support, to improve clinical outcomes and patient experience. The role has primary responsibility and oversight for population health programming including chronic care management, care coordination activities, preventive care, payer incentive programs including the accountable care organization and clinically integrated network, patient experience, and other initiatives that advance population health outcomes.
This position collaborates with physician, administrative, clinical, and operational leadership to align office standards and workflows with Cameron’s Mission, Vision, and Values.

JOB TITLE: Population Health Manager
SHIFT: 1.0 FTE, 80 Hours Bi-weekly, Full-time, Days. M-F.
Essential Functions
- Serves as the primary quality improvement contact and subject matter expert for Cameron Medical Group providers, leaders, and staff.
- Partners with physician and administrative leadership to develop highly reliable, operationally excellent, and patient-centered workflows.
- Demonstrates a results-oriented approach to problem-solving, change management, and continuous improvement while effectively navigating multiple priorities.
- Manages all payor incentive action plans, results, and improvement initiatives including accountable care organization, clinically integrated network, and additional incentive programs.
- Works closely with the Executive Director, clinic managers, providers, and staff to identify workflow opportunities and implement more efficient and effective processes.
- Provides education, coaching, and ongoing support to providers and staff regarding quality measures, clinical workflows, documentation expectations, and performance improvement opportunities.
- Supports the development, implementation, and evaluation of population health goals and strategies based on identified patient and community needs.
- Uses clinical, operational, payer, and patient data to identify population health trends, care gaps, health disparities, and opportunities for intervention.
- Develops and implements workflows to improve preventive care, chronic disease outcomes, access to care, patient engagement, and health equity.
- Creates and maintains dashboards that identify trends, successes, and opportunities related to population health, patient experience, clinical quality, utilization, and other key performance indicators.
- Supports patient outreach and engagement strategies for patients who are overdue for preventive services, chronic disease follow-up, or other recommended care.
- Promotes social determinants of health data collection and collaborates with care coordination and community partners to address barriers affecting patient outcomes.
- Promotes a strong emphasis on patient experience, engagement, access, and the overall experience in the ambulatory setting.
- Is the subject matter expert for patient experience; analyzes and disperses patient experience results and feedback to identify trends and improvement opportunities.
- Supports the overall patient experience strategy across Cameron Health.
- Incorporates the patient and family perspective into quality improvement and workflow design when appropriate.
- Partners with clinic leadership and staff to develop and implement strategies that improve communication, service, access, and patient-centered care.
- Facilitates quality improvement activities using QAPI, Lean, project management, or other structured improvement methodologies.
Qualifications
Experience
- Minimum of five years healthcare experience required.
- Minimum of three years Lean and/or project management experience preferred.
- Minimum three years incentive-based knowledge preferred.
- Minimum three years patient experience engagement preferred.
- Minimum three years ambulatory clinical integration (CI) experience preferred.
- Epic experience preferred.
