Job Overview

We are seeking a dedicated and experienced Medical Assistant with expertise in Chronic Care Management (CCM) to join our healthcare team in Dallas, TX. The ideal candidate will have hands-on experience supporting patients with chronic conditions, assisting providers in delivering comprehensive care, and ensuring effective management of chronic illnesses.

Key Responsibilities

• Assist healthcare providers in managing patients with chronic conditions such as diabetes, hypertension, COPD, and others.
• Perform patient intake, including vital signs, medical history updates, and medication reconciliation.
• Coordinate and document Chronic Care Management services in compliance with CCM guidelines.
• Educate patients on disease management, medication adherence, and lifestyle modifications.
• Schedule follow-up appointments and facilitate communication between patients and providers.
• Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
• Support care coordination efforts to improve patient outcomes and reduce hospital readmissions.

Qualifications

• Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) preferred.
• Proven experience in Chronic Care Management (CCM) within a clinical setting.
• Strong knowledge of chronic disease processes and management protocols.
• Excellent communication and patient education skills.
• Familiarity with electronic health records (EHR) and documentation standards.
• Ability to work collaboratively in a fast-paced healthcare environment.
• High level of professionalism and attention to detail.
