Position Summary

Impact Health PC is seeking an experienced Medical Director and/or Attending Physician to provide care within Skilled Nursing Facilities in the Hartford, CT area.

This position offers the opportunity to provide direct patient care while also serving in a clinical leadership capacity. The Medical Director works closely with facility leadership, attending providers, nursing staff, and interdisciplinary teams to promote high-quality care, regulatory compliance, and continuous clinical improvement.

Physicians may have the opportunity to serve as an Attending Physician, Medical Director, or a combination of both, depending on experience, availability, and facility needs.

Key Responsibilities

• Provide high-quality medical care to residents within Skilled Nursing Facilities.

• Serve as an Attending Physician and/or Medical Director based on facility needs.

• Provide clinical leadership and medical oversight to support quality patient care.

• Collaborate with facility leadership, nursing staff, advanced practice providers, specialists, and interdisciplinary care teams.

• Review patient records, clinical outcomes, adverse events, infection control initiatives, and quality metrics.

• Participate in and provide oversight for Quality Assurance and Performance Improvement (QAPI) initiatives.

• Ensure care is provided in accordance with applicable federal, state, and facility regulations.

• Provide evidence-based recommendations regarding diagnoses, treatment plans, and overall patient management.

• Maintain appropriate on-site presence and remain available for clinical consultation as needed.

• Arrange appropriate physician coverage when unavailable.
