Job Summary

The Medical Records Clerk is responsible for compiling, organizing, and maintaining patient medical records to ensure accurate documentation of conditions and treatments. Responsibilities include reviewing records for completeness, abstracting and coding clinical data using standard classification systems, and compiling medical and census data for reporting. The clerk also responds to inquiries from law firms, insurance companies, and government agencies; maintains health record indexes and retrieval systems; and uses computer systems to process, store, and retrieve health information.


Essential Functions

The following duties are considered essential functions. Reasonable accommodations may be made in accordance with the ADA to enable individuals with disabilities to perform these functions.

  • Analyze patient medical records for compliance with federal and state regulations and organizational standards.
  • Identify documentation deficiencies and ensure timely correction.
  • Document the transfer of information to physicians, hospitals, law firms, and health departments.
  • Pull, file, and refile medical records as needed.
  • Assist with patient registration and answer incoming telephone calls.
  • Create medical records for new patients.
  • Insert laboratory results and other clinical documents into patient charts.
  • Deliver and retrieve correspondence for providers and nursing staff.
  • Perform related duties and administrative tasks as assigned.

Schedule

  • Monday–Friday, 8:00 am–5:00 am, with potential extended hours as needed.
  • This is an on‑site position.

Education and Experience

  • High school diploma or GED required.
  • Previous medical records experience preferred.
  • Strong time‑management and organizational skills.
  • Ability to build effective working relationships at all levels.
  • Strong interpersonal and communication skills.
  • Ability to prioritize tasks and manage workload efficiently.
  • Proficiency with computers and office equipment.
  • Ability to remain calm, professional, and tactful during stressful situations or emergencies.
