- Location: Evansville, IN
- Facility: Ascension Medical Group Clinic
- Department: Primary Care
- Schedule: Part Time | 8am - 4:30pm | 3 days one week and 2 days the second week
- This position will support two clinics: 100 S. Rosenberger Ave, Evansville, IN 47712 and Ascension Medical Group St. Vincent - Evansville Primary Care
- Lead comprehensive Annual Wellness Visits for Medicare patients, translating complex preventive health guidelines into clear, personalized care plans that help individuals stay healthier, longer.
- Champion patient advocacy and care coordination across multidisciplinary teams, proactively identifying barriers to care and connecting patients with vital community resources.
- Drive proactive pre-visit planning to ensure all essential preventive screenings, immunizations, and health maintenance goals are identified and scheduled before the patient walks through the door.
- Partner seamlessly with physicians and care teams to maintain continuity of care, exercising sound clinical judgment while building trust with patients and their families.
- Deliver thorough, high-quality clinical documentation in the electronic medical record, ensuring every patient interaction accurately reflects the care journey and satisfies national quality and compliance standards.
- Required Credential(s):
- Registered Nurse obtained prior to hire date or job transfer date.
- BLS Provider. American Heart Association or American Red Cross accepted.
- Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
