Mental Health Case Manager

About the Position: This position is located at Whiteman Air Force Base, Missouri. This is a Direct Hire Solicitation

Serve as expert mental health case manager and member of multidisciplinary team of medical and professionals to identify high risk and high-cost diagnosis.

Facilitate optimal patient outcomes by aligning treatment plans with best practices and the unique needs of each patient.

Design timeline-based care protocols that ensure high-quality, cost-effective healthcare for patients and beneficiaries under case management.

Observe and evaluate patient behavior and responses, updating treatment plans in consultation with the healthcare team.

Monitor, track and intervene as patients transition between levels of care.

Serve as a liaison with local, state, and federal agencies to gather regulations, policy documentation, and resource materials, contributing to the development of procedural guidelines that improve patient access to existing programs.
