Mental Health Case Manager
About the Position: This position is located at Naval Medical Center San Diego with Directorate of Mental Health. Facilitate optimal patient outcomes by aligning treatment plans with best practices and the unique needs of each patient.
Track client progress towards goals, modifies care plans as needed, and advocates for clients' rights and preferences.
Observe and evaluate patient behavior and responses, updating treatment plans in consultation with the healthcare team.
Monitor psychotropic medication use, tracks physical and emotional side effects, recommends adjustments to therapy modalities, appropriate level of care placement, and provide clinical feedback to providers.
Work with mental health clinic staff to ensure patient needs are met based upon the patient’s assessed condition.
Collect and analyze data to identify trends and patterns and makes recommendations for program enhancements.
Coordinate medical services needed to optimize current health, psycho-social, spiritual status, and benefits.
Conduct training, education, counseling, and clinical assistance in mental health case management techniques to nursing personnel, interns/residents, and administrators.
