Mental Health Case Manager
About the Position: This position is located at General Leonard Wood Community Hospital's Mental Health Services in Fort Leonard Wood, Missouri. A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. This is a Direct Hire Solicitation
Monitors, tracks and intervenes as patients transition between levels of care.
Develops and recommend strategies to improve healthcare access.
Coordinates the weekly At-Risk/Multi-Disciplinary Treatment Planning (ARCT/MDTP) meeting.
Develops, implements, communicates, and coordinates individualized empirically informed patient treatment plans.
Serves as expert mental health case manager and member of multidisciplinary team of medical and professionals.
Schedule and monitor medical appointments, therapy sessions, and support services and ensure continuity of care across multiple systems.
Assess, develop and recommend local operating procedures to meet needs of the clinic, Medical Treatment Facility and population served.
