Mental Health Case Manager
About the Position: This position is located at the Naval Hospital Bremerton within the Directorate of Mental Health in Kitsap, Washington. A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. Non-Federal work experience may be creditable for leave purposes. This is a Direct Hire Solicitation
Serves as expert mental health case manager and member of multidisciplinary team of medical and professionals to identify high risk and high-cost diagnosis.
Facilitate optimal patient outcomes by aligning treatment plans with best practices and the unique needs of each patient, providing individual and group support,
psychoeducation, and skills training on topics such as stress management coping mechanisms, and relapse prevention.
Develop, implement, communicate and coordinate individualized empirically informed patient treatment plans that address all diagnosed conditions and health concerns.
Monitor psychotropic medication use, tracks physical and emotional side effects, recommends adjustments to therapy modalities, appropriate level of care placement, and provide clinical feedback to providers.
Participate in clinical audits and performance reviews to advance patient safety and care effectiveness.
Chairs and coordinates the weekly meetings to ensure treatment plans for at-risk and high-interest patients are synchronized among all medical treatment providers.
