Mental Health Case Manager

About the Position: This position is located at the 49TH Medical Group, Holloman Air Force Base, New Mexico. This is a Direct Hire Solicitation A recruitment or relocation incentive maybe authorized. Advanced in-hire maybe considered for those candidates who are new to Federal service. Advanced leave accrual MAY be authorized.

Serve as a behavioral health case manager on a multidisciplinary team, specializing in identifying high-risk and high-cost diagnoses.

Facilitate patient success through direct intervention and advocacy.

Observe patient behavior and response to treatment and adjusting levels of care in consultation with the broader healthcare team to ensure all diagnosed conditions are addressed.

Coordinate care between treating facilities, command, and medical teams (within DHA guidance), as well as integrating family support and personalized safety plans

Coordinates the weekly At-Risk/Multi-Disciplinary Treatment Planning (ARCT/MDTP) meeting.

Provide direct training, counseling, and clinical assistance to a wide range of personnel

Serve as a liaison for the Military Treatment Facility (MTF) with local, state, and federal agencies
