Summary
Provides direct mental health services to adults in community, home, or office settings, collaborating with a care team to develop treatment plans and facilitate individual/group sessions.

Our team is seeking a Mental Health Practitioner to provide direct mental health services to adults out in the community, at their home, or at the office. You will work collaboratively with a dedicated team of professionals to provide the highest quality of care possible.
Responsibilities
- Maintain a caseload of clients.
- Help develop and work will all members of the treatment plan team as often as needed.
- Facilitate services to clients individually and in a group setting.
- Coach, teach, educate, and model independent living skills to clients.
- Coordinate and make further referrals for clients as needed.
- Seek treatment consultation from Clinical Supervisor.
- Accurately completes all required documentation and notes for each client in a timely manner.
