Albany Care, a Specialized Mental Health Rehabilitation Facility (SMHRF), in Evanston, IL, has Immediate Openings for a Mental Health Professional (MHP) to join our team. These is s Full Time Position with 8 Hour Shifts.

Starting Rate of up to $28 per hour, depending on Benefits Selection!

Available Position:

FT - Tuesday-Saturday, 3pm-11pm

Mental Health Professionals (MHPs) work in conjunction with the interdisciplinary team, offering recovery and rehabilitation support and services to those who live in the facility.

Responsibilities of the Mental Health Professional (MHP)

• Collaborate with consumer and treatment team to complete assessments and develop therapeutic treatment plans
• Implement therapeutic care plans and activities in a compassionate and caring manner
• Recognize, note, and inform treatment team when a consumer is not at their baseline of symptoms and behaviors
• Lead groups and hold one-on-one sessions including engagement with evidenced based approaches
• Collaborate with community agencies to coordinate care, including the Moving On Program
• Crisis management and intervention·
• Effectively manage a caseload of up to 32 individuals
• Participation in creation of discharge plans and recommendations for other levels of care
