SHIELDS’ mission is to develop, deliver and evaluate culturally sensitive, comprehensive service models that empower and advocate for high-risk families in South Los Angeles.

The Plans of Safe Care (POSC) Navigator provides prenatal and postpartum support to women identified with substance misuse or substance use disorders through hospital and home visits. This role conducts assessments, enrolls participants, and collaborates with families and hospital staff to develop individualized Plans of Safe Care. Navigators provide education on prenatal and postnatal care, breastfeeding, parent–infant attachment, child development, home safety, and maternal and infant health. The position also identifies participant strengths, needs, and barriers while connecting families to behavioral health services, substance use treatment, home visitation programs, and other community resources. Navigators offer empathetic, family-centered support to mothers, infants, and families and participate in a countywide collaborative supporting the implementation of Plans of Safe Care in Los Angeles County.

Primary Duties:
Work closely with hospital social work teams to assess and enroll participants at the bedside and develop Plans of Safe Care in collaboration with families and care teams. Conduct prenatal and postpartum home visits, provide parenting and maternal health education, and coordinate referrals to appropriate services and community resources. Maintain supportive relationships with participants using a compassionate, family-centered approach. Ensure timely and accurate documentation, including screenings, assessments, progress notes, and POSC-related records. Coordinate services through case conferences and collaboration with referral sources such as the Department of Children and Family Services and community agencies. Collect and enter program data, maintain updated community resource information, generate reports, and submit monthly updates to the program manager. Attend required meetings and trainings, maintain compliance with agency and regulatory requirements, and support program events and activities as needed.
