Uplift HomeCare is seeking a licensed Physical Therapist (PT) to provide skilled in-home therapy services throughout Winter Haven and surrounding communities.

KEY RESPONSIBILITIES

• Perform physical therapy evaluations and develop individualized treatment plans.
• Provide skilled therapy interventions, including therapeutic exercises, gait training, balance training, and mobility training.
• Conduct reassessments and monitor patient progress toward established goals.
• Educate patients and caregivers on home exercises, fall prevention, and safe mobility.
• Coordinate care with physicians and other healthcare professionals.
• Complete required clinical documentation accurately and within established timeframes.

QUALIFICATIONS

• Active, unrestricted Florida Physical Therapist (PT) license.
• Home health experience preferred; requirements may vary by assignment.
• Medicare home health documentation and OASIS experience preferred, where applicable.
• Current CPR certification.
• Required background screening, as applicable.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong clinical, communication, and documentation skills.
• Ability to work independently and travel to patient homes.
