Key Responsibilities

• Evaluate patients' physical abilities, mobility, strength, balance, coordination, and functional limitations.
• Develop and implement individualized physical therapy treatment plans based on patient needs and established goals.
• Provide therapeutic exercises, gait training, balance training, transfer training, and mobility interventions.
• Monitor patient progress and modify treatment plans as appropriate.
• Educate patients and caregivers on home exercise programs, fall prevention, safe mobility, and proper use of assistive devices.
• Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate patient care.
• Complete physical therapy evaluations, reassessments, progress notes, discharge summaries, and other required clinical documentation accurately and on time.
• Follow applicable Medicare requirements, Florida regulations, infection-control standards, HIPAA guidelines, and agency policies.
• Help patients achieve their highest possible level of function while promoting safety, dignity, and independence.

Qualifications

• Current, unrestricted Florida Physical Therapist (PT) license.
• Home health experience preferred.
• Familiarity with Medicare home health documentation and OASIS-related processes preferred, where applicable.
• Experience with Axxess or another home health electronic medical record (EMR) system is a plus.
• Current CPR certification.
• AHCA Level II background screening, as required.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong clinical, communication, organizational, and time-management skills.
• Ability to work independently while collaborating effectively with the healthcare team.
• Commitment to compassionate, high-quality patient care.
