Nurse (Clinical/Ambulatory)

About the Position: This position serves as a Nurse (Clinical/Ambulatory) performing independent and comprehensive triage for patients, facilitate coordination with multi-disciplinary teams to provide the highest level of care, and provide education to patients. It is located at Womack Army Medical Center, Family Medicine at Fort Bragg, NC. This is a Direct Hire Solicitation Salary negotiation may be available for those candidates who are new to Federal service.

Perform independent, comprehensive telephone and clinical triaging, and facilitates care coordination through a multidisciplinary approach.

Act as relief nurse, triages and screens walk-in patients to assess the severity of the patient's condition and the need for provider intervention.

Screen patient charts to assess the medical history and determine health care needs.

Assist clinic with patient and staff education.

Coordinate patient referrals to support services provided through the internal and external community, or the TRICARE network.

Develop appropriate nursing plans.

Follow quality improvement and internal control activities for the ambulatory care setting to include risk appraisals, conducting chart reviews, identifying trends and reporting corrective actions.
