Nurse (Clinical/Case Management)

About the Position: This position is located at Naval Health Clinic New England, Newport, Rhode Island. Salary negotiation may be available for those candidates who are new to Federal service. A recruitment or relocation incentive may be authorized. Non-Federal work experience may be creditable for leave purposes. This is a Direct Hire Solicitation

Attends daily multi-disciplinary team meetings to address current issues, and improve the overall process, and identify potential case management patients review of the multiple databases and points of contacts available for the specified population.

Completes follow up assessments in the electronic record with updated information of the patient's current physical and emotional condition.

Coordinates services, referrals, and appointments for each patient under case management services.

Conduct assessment and identify needs to formulate care plans for patients with a variety of complex health concerns, which includes researching, analyzing, testing, and evaluating information, uncommon situations, or unpredictable conditions.

Serves as liaison between the patient and the health care professionals.

Conducts a comprehensive assessment of beneficiary’s health needs to develop a plan of care.

Coordinates services needed by patients with a variety of complexities and provides intervention when the patient deviates from the anticipated path of progression.
