Nurse (Consultant/MHPT/PCMH)

About the Position: Incumbent assists in the development, monitoring, and managing of healthcare delivery systems, associated with the Medical Home Port Team (MHPT) and the Patient Centered Medical Home (PCMH), through communication and use of resources. This is a Direct Hire Solicitation

Assess patient condition based upon observation, examination and/or interview, physician's diagnosis, laboratory findings, age-related issues, and other pertinent information.

Identify, document, implement, and formulate the patient care plan as determined by the patient's condition.

Calculate drug dosages and administer intravenous fluids and medications using age-appropriate guidelines.

Respond to life-saving situations, initiating emergency resuscitation measures, according to pediatric and adult resuscitation protocols.

Provide education and support to patients and their family members.

Demonstrate clinical expertise in the execution of the nursing process and participates with staff in its application.

Utilize problem-solving techniques to ensure the delivery of quality patient care.

Encourage patients and their family to actively participate in patient health care needs.
