Nurse (Consultant/MHPT/PCMH)

About the Position: Incumbent assists in the development, monitoring, and managing of healthcare delivery systems, associated with the Medical Home Port Team (MHPT) and the Patient Centered Medical Home (PCMH), through communication and use of resources. This is a Direct Hire Authority (DHA) solicitation utilizing the DHA for Certain Personnel of the DoD to recruit and appoint qualified candidates to positions in the competitive service.

Assess patient condition based upon observation, examination and/or interview, physician's diagnosis, laboratory findings, age-related issues, and other pertinent information.

Identify, document, implement, and formulate the patient care plan as determined by the patient's condition.

Calculate drug dosages and administer intravenous fluids and medications using age-appropriate guidelines.

Respond to life-saving situations, initiating emergency resuscitation measures, according to pediatric and adult resuscitation protocols.

Provide education and support to patients and their family members.
