Nurse (Mental Health Care Facilitator)

About the Position: This position is located at Captain James A. Lovell Federal Health Care Center (FHCC) in North Chicago, Illinois. This is a Direct Hire Authority (DHA) solicitation utilizing the DHA for Certain Personnel of the DoD to recruit and appoint qualified candidates to positions in the competitive service.

Perform independent, comprehensive telephone and clinical triaging in the primary care setting.

Facilitate continuity of care through a multidisciplinary approach in the treatment of depression, post-traumatic stress disorder (PTSD) and anxiety disorders.

Monitor patient progress and symptom severity by conducting a structured telephonic interview and using validated tools.

Provide patient education as an integral part of the services offered.

Assist clinic with patient and staff education and provides individual and small group training.

Compile and maintain a variety of data reports, submitting detailed and aggregate program and caseload data reports as needed by program staff.
