Nurse (Mental Health Care Facilitator)

About the Position: This position is located at Kimbrough Ambulatory Care Center, Fort Meade. Serves as a Mental Health Care Facilitator that works with mental health providers to deliver high-quality patient care and facility operations. This is a Direct Hire Authority (DHA) solicitation utilizing the DHA for Certain Personnel of the DoD to recruit and appoint qualified candidates to positions in the competitive service.

Perform clinical triaging in the primary care setting.

Support primary care providers in treatment of patients with mental health conditions.

Facilitate continuity of care through a multidisciplinary approach.

Provide patient education as an integral part of the services offered.

Assist patients with treatment plan adherence.

Communicate with providers regarding patient status.

Assist clinic with patient and staff education.

Provide individual and small group training.

Engage in peer review of chart documentation.
