Nurse Practitioner
Description
The Nurse Practitioner (NP) is a Master's-prepared, independently licensed healthcare provider who delivers comprehensive primary care services within the Merakey Total Health community-based outpatient clinic. Working collaboratively with physicians and an interdisciplinary care team, the NP provides patient-centered, evidence-based care that supports the health and wellness of individuals across the lifespan.
This role requires strong clinical expertise, sound judgment, and a commitment to integrated, community-focused care. The NP partners with healthcare providers, behavioral health professionals, and community resources to improve health outcomes, promote continuity of care, and address the unique needs of the populations served.
Key Responsibilities include:
- Clinical Assessment & Patient Care: Conduct comprehensive patient assessments, including medical histories and physical examinations; diagnose and manage acute and chronic health conditions; prescribe medications; develop individualized treatment plans; and monitor patient outcomes, adjusting care plans as needed to support optimal health outcomes.
- Diagnostic Evaluation & Treatment Planning: Order, perform, and interpret diagnostic tests, laboratory studies, and other clinical evaluations; utilize findings to develop and implement evidence-based treatment recommendations.
- Patient & Family Education: Educate patients and families regarding diagnoses, treatment plans, medications, preventive care, and wellness strategies; empower patients to effectively manage their health conditions and achieve positive outcomes.
- Documentation & Compliance: Maintain timely, accurate, and thorough documentation within the electronic health record (EHR), including progress notes, treatment plans, and clinical summaries; ensure compliance with organizational, regulatory, and quality standards.
- Care Coordination & Collaboration: Collaborate with physicians and multidisciplinary team members to coordinate and optimize patient care; facilitate continuity of care through effective communication with internal and external healthcare providers.
- Leadership, Quality & Community Engagement: Serve as a clinical resource for staff; contribute to quality improvement, patient safety, and best-practice initiatives; build and maintain partnerships with community healthcare providers and organizations to improve access to care and support continuity of services.
