Job Summary

The PACE Nurse Practitioner delivers direct primary medical care to PACE participants, conducting comprehensive initial and ongoing assessments as an integral member of the Interdisciplinary Team care planning process. The NP manages chronic and acute health conditions under the supervision of the Medical Director or Primary Care Physician, initiates evidence-based interventions, and provides primary care education to participants, caregivers, and staff to promote optimal health outcomes.
