You'll handle everything from acute changes in condition to long-term care planning.

  • Manage acute changes in condition, chronic conditions and advanced illness
  • Lead advance care planning conversations with residents and families

  • Work with nursing staff, interdisciplinary teams and collaborating primary care physicians
  • Keep accurate, timely documentation in the EMR

  • Active, unrestricted Nurse Practitioner license in Ohio
  • NP board certification (adult, family or geriatric) and an active DEA registration
  • 1+ years of experience as a practicing Nurse Practitioner
  • Clinical experience in long-term care, a nursing home or a skilled nursing facility
  • Comfortable working in an EMR and as part of an interdisciplinary team
  • Able to work on-site 4 days a week and live within about an hour of the assigned facility
