OC FSP HFW Facilitator
PROGRAM SUMMARY
Beginning July of 2026, all Behavioral Health Plans in California will be required to implement High-Fidelity Wraparound (HFW) to fidelity under the Full Service Partnership (FSP) program. All FSPs for children and youth must align with the principles, values, and standards of the HFW model.
HFW provides a comprehensive, holistic, evidence-based, child/youth and family-driven process for responding when children and youth experience significant mental health or behavioral challenges, including children and youth involved in multiple child/youth serving systems. HFW is a team-based and family-centered evidence-based practice that includes an "anything necessary" approach to care for children and youth with the most intensive mental health or behavioral challenges
POSITION SUMMARY
The Facilitator ensures that the Wraparound process is implemented with fidelity and integrity, consistently maintaining hope that each family can achieve a better future. This role is responsible for initiating engagement with parents and youth, forming the Child and Family Team (CFT), developing safety plans, and creating an individualized CFT plan based on family strengths, needs, and preferences.
Throughout the process, the Facilitator maintains a non-judgmental, strengths-based, and culturally responsive approach. They coordinate the development and ongoing revision of CFT plans, reflecting lessons learned from what has and has not worked. From the beginning of care, they also plan for transitions, ensuring smooth movement toward independence and stability.
The Facilitator provides leadership to the team, consistently demonstrating Wraparound values and principles. They engage others in a shared mission, display strong interpersonal skills, and effectively navigate the complex systems of public services. They also build strong connections among CFT members, valuing diverse perspectives that support family-driven success. Additionally, the Facilitator provides direct services and interventions aligned with the Wraparound plan. This position may be expected to participate in an on-call rotation.
GENERAL REQUIREMENTS:
Education/Experience
- Bachelor’s Degree in social work, psychology, nursing, occupational therapy, or a related field from an accredited college or university preferred.
- One year experience in human services, preferably case management.
- Must qualify as a Medi-Cal Specialty Mental Health Services (SMHS) provider. Eligible activities may be claimed under Medi-Cal if all state and federal Medicaid requirements are met.
- Required Training - Completion of the Statewide Standardized Child and Family Team (CFT) Facilitation Training, accessible through Regional Training Academies, the UC Davis Resource Center for Family-Focused Practice (RCFFP), or CACWT
Licensure, Registration, Certification
- Valid CA driver’s license and proof of automobile insurance.
- Completion of CPR/First Aid/ProACT certification within the first thirty (30) days of employment.
Preferred training/experience in the following Evidenced Based Practices:
- Intensive Case Management (ICM)
- High Fidelity Wraparound (HFW)
