Duties
- Conduct thorough assessments of patients' health conditions and needs.
- Develop and implement individualized care plans based on patient assessments.
- Collaborate with clinical managers and referral sources to build appropriate plans of care.
- Monitor clinical performance metrics to optimize patient outcomes.
- Complete daily workflow
- Comply with state and federal regulatory guidelines.
- Work in a team environment with operational support and sales team.
- Assist with weekend assessments, on rotation basis.
- Provide other duties as needed.
