JOB PURPOSE:

The Occupational Therapist provides occupational therapy services to patients in the home setting.

Work includes responsibility for the assessment, planning, implementation, and evaluation of patient’s

occupational therapy plan of treatment. Salary range posted is for a full time position.

QUALIFICATIONS:

• Minimum of one year practicing as an Occupational Therapist.
• Possess and maintain a current license as an Occupational Therapist from the State Board of Occupational Therapy Examiners.

RESPONSIBILITIES:

• Conducts a complete physical evaluation in order to determine appropriate treatment in accordance with the functional level of the patient.
• Obtains a complete history through interview of patient/family/others.
• Plans, develops, implements, and evaluates occupational therapy treatment for patients in accordance with the physician’s plan of care and the patient’s abilities/limitations.
• Completes a plan of care that is thorough with measurable short and long-term goals.
• Teaches home occupational therapy programs to patients and/or their families and assists them in planning/implementing equipment needs for home use.
• Instructs patients in specific skills such as independent daily living skills, grooming and hygiene, sensorimotor, and social interaction.
• Regularly reevaluates the rehabilitation needs of the client and modifies occupational therapy programs based upon those needs.
• Observes signs and symptoms and reports to the physician and/or other appropriate health care professionals as often as needed or as changes in the patient’s condition requires
