Position Summary
The Occupational Therapist Registered (OTR) evaluates residents and develops, implements, and updates individualized occupational therapy plans of care to help residents achieve maximum independence and quality of life.
Responsibilities may include providing care in the following settings:
- Skilled Nursing Facility (SNF)
- Home Health Care

Key Responsibilities
- Perform occupational therapy evaluations and establish individualized plans of care.
- Provide skilled occupational therapy treatments in accordance with physician orders.
- Supervise and collaborate with Certified Occupational Therapy Assistants (COTAs), as applicable.
- Accurately document evaluations, treatments, resident progress, and discharge plans.
- Participate in interdisciplinary care planning and team communication.
- Ensure compliance with facility policies, professional standards, and state and federal regulations.
