We are currently seeking a Full-Time Registered Occupational Therapist (OTR) to join our skilled nursing team. The ideal candidate is compassionate, organized, and committed to helping residents regain independence and improve their quality of life.
**Must be willing to relocate to Phoenix, AZ

Key Responsibilities
- Conduct occupational therapy evaluations and develop individualized treatment plans
- Provide skilled OT services to residents in a skilled nursing setting
- Assist residents with activities of daily living (ADLs), mobility, and functional independence
- Collaborate with an interdisciplinary team to support resident-centered care
- Complete timely and accurate documentation in accordance with Medicare and facility guidelines
- Participate in care planning and discharge planning as needed
Qualifications
- Active Occupational Therapist (OTR) license in the state of practice
- Graduate of an accredited Occupational Therapy program
- SNF or post-acute experience preferred
- Strong communication and documentation skills
- Ability to manage a caseload effectively in a fast-paced environment
