Physician Affiliate Group of New York (PAGNY) is adding an Optometrist (Per Diem) to the group at NYC Health + Hospitals/Administration for Children's Services. PAGNY staffs several municipal hospitals and clinics within New York City through an affiliation agreement with NYC Health + Hospitals. PAGNY’s corporate staff provides administrative, practice management, and human resources services in support of its more than 4,000 clinical providers.
The HEAL Program is a new NYC Health + Hospitals initiative to transition into being the primary provider of medical services to young people in City custody. The staff will be responsible for 24/7 medical care across two secure detention sites, two school-based health centers, and limited pharmacy services at seven non-secure sites (group homes). Services include comprehensive medical screenings, dental screenings and procedures, vaccinations and routine testing, responses to immediate need, and referrals for offsite care within and outside the NYC Health + Hospitals system.

Opportunity Details:
- Routine on-site eye examinations including problem-oriented history, visual acuity, eye health assessment, refraction, disposition, and eyeglass order generation.
- Completion of any documentation, records, and/or any other vision services, within the equipment parameters and scope of Optometrists, as provided by the State.
- Referral to an Ophthalmologist, other physician(s), or telemedicine services for care beyond the scope of on-site Optometry, as needed.
