Outreach Counselor- CSP

What You Will Do:

  • Provide in-person, telephone, and telehealth outreach and care-management support to individuals with behavioral health and/or substance-use needs, helping them overcome barriers and connect with essential services.
  • Serve as the primary point of contact for an assigned caseload, developing strong therapeutic rapport and coordinating responsive, person-centered support.
  • Meet with individuals admitted to hospitals, substance-use treatment programs, and other 24-hour settings and maintain contact throughout their stay to support discharge planning and successful community re-engagement.
  • Complete biopsychosocial assessments, Individualized Action Plans, Safety Plans, and Disenrollment Plans collaboratively with individuals, providers, and supports under clinical supervision.
  • Coordinate care with probation and parole officers, primary-care providers, nurses, behavioral health and substance-use providers, and natural supports as authorized by the individual.
  • Help individuals access health insurance, food and nutrition assistance, housing placement and stabilization, financial and utility assistance, education, employment, legal support, and other community resources.
  • Promote improved health, independence, self-sufficiency, community stability, and reduced risk of future criminal-justice involvement through integrated and recovery-oriented care management.
  • Maintain professional, collaborative relationships with providers, justice-system partners, community agencies, and other supports involved in each individual's care.
  • Complete timely and accurate documentation, maintain program productivity expectations, attend required meetings and training, and perform other duties assigned by program leadership.

Who You Are:

  • High school diploma or GED required.
  • At least one to two years of related experience in behavioral health, substance-use services, corrections, care management, or another human-services field required.
  • Community Health Worker certification and/or a bachelor's degree preferred.
  • Bilingual Spanish and English communication skills preferred.
  • Knowledge of mental health and substance-use disorders, care-management practices, community resources, and the needs of justice-involved populations.
  • Experience collaborating with justice-system entities and helping individuals navigate healthcare, behavioral health, social-service, and community systems.
  • Lived experience with the criminal-justice system and recovery from behavioral health or substance-use challenges is valued and preferred.
  • Strong interpersonal, cultural-responsiveness, verbal, and written communication skills, with the ability to establish trust and maintain professional boundaries.
  • Strong organization, time-management, documentation, technology, and follow-through skills, including the ability to manage multiple priorities and urgent needs.
  • Valid driver's license and a registered, inspected, and reliable vehicle available during all working hours required.
  • Ability to meet the physical requirements of the role, including community-based activity and lifting or carrying up to 25 pounds, with or without reasonable accommodation.
