Outreach, Enrollment & Provider Network Manager
- Collaborate with clinical operations and IDT teams on care transitions
- Coordinate centralized referral workflows and external appointment scheduling
- Coordinate delegated credentialing and compliance requirements
- Coordinate intake staffing to meet monthly enrollment targets
- Ensure specialty scheduling visit note retrieval and participant follow through
- Escalate enrollment issues to maintain eligibility continuity
- Handle provider relations issue resolution and escalations
- Maintain training materials and job aids for intake workflows
- Manage participant enrollment and disenrollment workflows
- Monitor network adequacy and service level concerns
- Perform OIG exclusion checks and required attestations monitoring
- Supervise coach and develop intake and outreach coordinators
- Support provider network operations including provider data maintenance credentialing coordination and access management
- Support workforce and process improvement initiatives
- Track CMS and DHCS submission and documentation
- Track provider contract compliance including medical record return timeliness
- Verify Medi Cal and Medicare eligibility and redetermination
