GENERAL STATEMENT OF DUTIES:

The Elevations Clinic Outreach Specialist is responsible for increasing awareness of clinic services, developing community partnerships, and improving access to behavioral health services for individuals and families. The Outreach Specialist builds relationships with community organizations and referral sources, coordinates referrals, supports client engagement, and assists individuals in connecting with appropriate clinic and community resources.

KEY ESSENTIAL FUNCTIONS:

Community Outreach & Engagement

  • Develop and maintain relationships with victim service providers, healthcare and behavioral health providers, schools, shelters, public agencies, community-based organizations, and other referral sources throughout Suffolk County.
  • Participate in community events, meetings, presentations, workshops, and other outreach activities to increase awareness of Elevations Clinic services.
  • Provide information about clinic services and available community resources to individuals, families, and community partners.
  • Assist with the development and distribution of approved outreach materials and informational resources.

Referral & Client Engagement

  • Coordinate and follow up on referrals to support timely connection to appropriate clinic services.
  • Conduct outreach to individuals who have not engaged with or have disengaged from services, as appropriate.
  • Assist individuals and families with connecting to appropriate clinic and community resources.
  • Assist clients with accessing available victim-related benefits, compensation, and other services, including providing information and assistance with applications as appropriate.
  • Work collaboratively with the Clinical Therapist and Peer Support Specialist to support client engagement, referrals, and community linkages while maintaining appropriate role boundaries.

Documentation & Compliance

  • Document outreach activities, referrals, contacts, and outcomes in the designated electronic record or tracking system.
  • Maintain an updated community resource directory and provide outreach reports as requested.
  • Maintain compliance with HIPAA, confidentiality standards, and agency policies.

QUALIFICATIONS:

Education & Experience

  • Bachelor’s degree in social work, psychology, human services, or a related field preferred.
  • Experience working with community resources, referral networks, clients, and families preferred.
  • Experience with community outreach and partnership development preferred.
  • Experience working with diverse populations preferred.

Skills & Requirements

  • Strong interpersonal, communication, organizational, and relationship-building skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Ability to maintain appropriate professional boundaries and client confidentiality.
  • Proficient in Microsoft Office, email, and electronic record systems.
  • Valid NYS Driver’s License.
  • Bilingual (Spanish-speaking) is a plus.
  • Ability to travel throughout Suffolk County for community outreach activities.
  • Other duties as assigned.
