Outreach Worker- AOD (3441)
Description
Reduce barriers. Support healthier futures.
As a Street Health Outreach Worker specializing in Alcohol and Other Drug Services at Father Joe’s Villages, you will work directly with individuals experiencing homelessness and substance use challenges, helping them access the care, resources, and support they need to move toward greater stability. Through street-based outreach, harm reduction, and Enhanced Care Management (ECM), you will build relationships, address immediate needs, and help individuals navigate healthcare, substance use treatment, housing, and other essential services.
Working in the community and alongside healthcare providers, case managers, and community partners, you will maintain an assigned caseload, coordinate individualized care plans, and help clients overcome barriers to accessing services. Whether providing harm-reduction education, connecting someone to treatment, or following up on a healthcare referral, you will support clients’ progress toward their goals.
