Palliative Care LPN

JOB FUNCTIONS:

• Maintain records, reports and forms that are accurate, timely, and contain quality content per documentation policies.
• Make weekly and biweekly phone calls and home visits as needed and document all calls/visits in EMR.
• Participate in IDT meetings.
• Interact with Palliative Care Inter-Disciplinary Team (IDT) to collaborate on plan of care and needs of the patient and patient’s family.
• Assist team members with monitoring the program and meeting all measurable goals.
• Ensure quality and efficiency of patient palliative care services.
• Provide high-quality nursing care to patients, and work with families, relatives, and/or other primary care persons to enable them to become dependable caregivers.
• Perform any additional duties requested by the Palliative Care Coordinator and/or CEO.
• Educate the community on the Palliative Care Program as needed.
