Palliative Care Manager

Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Schedule: Monday–Friday, 8:30 a.m.–5:00 p.m.
Salary: $120,000–$145,000 annually
Employment: Full-Time
- Lead the day-to-day clinical operations of the Adult Palliative Care Program.
- Coach, support, and develop clinical team members while fostering a collaborative and accountable culture.
- Partner with the VP of Palliative Care on the strategic growth and development of the program.
- Help design and implement clinical workflows, policies, protocols, and processes that support high-quality care and sustainable growth.
- Coordinate patient referrals and ensure timely assignment, appropriate staffing, and continuity of care.
- Monitor clinical quality, patient outcomes, documentation, and regulatory compliance.
- Collaborate with hospitals, healthcare organizations, facilities, contracted partners, and referral sources to strengthen care coordination and access to Palliative Care.
- Identify opportunities to improve the patient and family experience throughout the Palliative Care journey.
- Participate in interdisciplinary collaboration and help solve complex clinical and operational challenges.
- Support staff education, professional development, and clinical excellence.
- Participate in the Administrative On-Call rotation.
- Supervising and developing clinical team members.
- Providing clinical leadership and mentorship to nursing staff.
- Providing administrative oversight to interdisciplinary team members, as applicable.
- Supporting staffing, workload management, and clinical coverage.
- Creating an environment of accountability, collaboration, and continuous improvement.
- Current, unrestricted California Registered Nurse (RN) license.
- Minimum of two years of RN experience in palliative care and/or hospice within the past five years.
- Three or more years of supervisory or healthcare leadership experience within the past five years.
- BSN preferred.
- Experience leading interdisciplinary healthcare teams.
- Strong communication, coaching, and relationship-building skills.
- Experience improving clinical workflows, quality, or program performance.
- Interest in helping grow and develop a community-based Palliative Care program.
