About the Role:

In this role, you will support parents as they navigate local systems and their own family dynamics, helping them access resources, build confidence, and take the lead in their plan of care. In this community‑based role, you’ll mentor families, advocate on their behalf, and work with a dedicated team to help them thrive.

What You’ll Do

• Partner with families and support teams to develop strength‑based, family‑driven plans of care
• Help families access community resources across the Greater New Haven area
• Participate in child and family team meetings and ensure ongoing engagement
• Advocate for families across various systems, providers, and service settings
• Mentor, educate, and empower parents to take an active leadership role in their own care plans
• Collaborate closely with the Family Support Specialist and other program staff
• Transport clients in your own vehicle when needed
