POSITION SUMMARY:

Responsible for evaluating and admitting patients into hospice care, ensuring they meet eligibility criteria and understand the services provided. This role involves conducting comprehensive clinical assessments, coordinating with physicians and the interdisciplinary team, and completing all required documentation accurately and timely.

Serves as a key point of contact for patients, families, and referral sources—providing education on hospice philosophy, goals of care, and end-of-life support. The Admissions Nurse also facilitates a smooth transition into hospice by initiating care plans and ensuring all services are in place in different care settings.

DUTIES AND RESPONSIBILITIES:

• Assesses patients for hospice eligibility, coordinates admissions, and ensures timely, accurate documentation.
• Educate patients and families on hospice care, collaborate with the care team, and help ensure a smooth transition into services with a strong focus on compassionate, patient-centered care.
• Promote the mission and goals of Hospice Austin through quality leadership and adaptability.
• Participate in and lead performance improvement initiatives to enhance patient care and team effectiveness.
