Part Time Speech Language Pathologist

We are seeking a compassionate and skilled Part-Time Speech Language Pathologist to join our home health team. The Speech Language Pathologist will provide high-quality, patient-centered speech and language therapy services to individuals in their homes. This role focuses on evaluating and treating communication and swallowing disorders, while collaborating with patients, families, and interdisciplinary care teams to support optimal outcomes.

Key Responsibilities:

• Conduct comprehensive assessments of speech, language, cognitive communication, and swallowing disorders in a home setting
• Develop individualized treatment plans based on patient needs, goals, and physician orders
• Provide evidence-based therapeutic interventions to improve communication, cognition, and/or swallowing function
• Educate patients, caregivers, and family members on therapy techniques, home exercises, and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Maintain accurate, timely documentation in compliance with regulatory and organizational standards
• Collaborate with physicians, nurses, occupational therapists, physical therapists, and other healthcare professionals
• Participate in care planning meetings and case conferences as required
