Shift: 10:00 PM - 7:00 AM

Join a dedicated home care team where your clinical skills directly improve patients’ lives in their own homes. As a Home Care LPN, you will provide direct and skilled nursing care under the supervision of an RN Case Manager, following individualized care plans tailored to each patient’s needs. You will deliver evidence-based care aligned with best practices, supporting improved health outcomes, safety, and a compassionate, high-quality patient experience.

Responsibilities

• Provide skilled in-home nursing care through comprehensive patient assessments and ongoing monitoring aligned with individualized care plans.
• Deliver safe, high-quality interventions as directed by the RN Case Manager, supporting optimal patient outcomes in the home setting.
• Educate patients, families, and caregivers on diagnosis, safety, and self-care strategies to promote independence and prevent complications.
• Coordinate closely with RNs and interdisciplinary team members to ensure seamless communication and continuity of care.
• Identify early signs of clinical decline and take proactive steps to reduce hospitalizations and avoid adverse events.

Qualifications

Education:

• Graduate of State Board approved program for Licensed Practical Nurses.

Experience:

• One (1) year of nursing experience caring for similar patient population as to be assigned.

Licensure/Certifications:

• Valid driver’s license.
• Proof of Auto Insurance.
• Valid LPN license in the state(s) of practice, including eligibility under an NLC multistate.
