Description

Cabin Creek Health Systems (CCHS) is recruiting a new floating pharmacist. This is a great opportunity to join the health care staff of a progressive Federally Qualified Health Center (FQHC). Pharmacists are respected as expert collaborators in the interprofessional patient care team model of integrated primary care (IPC) honored at CCHS.

At CCHS, you will have the opportunity to provide care to community members. This integration supports our mission to promote health and well-being to our patients.

As part of your involvement in collaborative patient care, you will have the opportunity to participate in regular interprofessional teamwork in various activities including team huddles, operations and systems planning, and problem solving at regular staff meetings and workgroup sessions. Pharmacists also have access to peer support and collaboration via quarterly pharmacy meetings and daily consultations through online networking across sites.

CCHS is the sponsoring agency for the Area Health Education Center and, as such, provides clinical preceptorships for health professional students (medical, nursing, psychology and pharmacy) and organizes regional continuing education functions. Pharmacists have the opportunity to supervise School of Pharmacy Pharm.D candidates seeking experience in the area of rural integrated primary care (IPC).
