Pharmacist (Outpatient Pharmacy, Medication Review, Flexible Schedule) #HDC1

A leading public healthcare group is looking for Pharmacists to join its outpatient pharmacy teams. You will deliver quality dispensing and pharmaceutical care across multiple clinics, with a flexible schedule to suit your availability.

What You'll Be Doing

• Provide quality dispensing service and pharmaceutical care to outpatients
• Review prescriptions for accuracy, appropriateness, dosage and potential drug interactions
• Provide Medication Review Service (MRS) to patients with chronic conditions
• Support Anticoagulation Service (ACS) and monitor patient outcomes
• Provide drug counselling to patients and caregivers
• Provide drug information to healthcare professionals and respond to enquiries from the public
• Collaborate with doctors, nurses and pharmacy technicians to ensure safe medication use
• Maintain accurate records and comply with pharmacy regulations and quality standards
• Support stock management, labelling and medication safety checks
• Cover multiple clinic locations as required by service needs

What You'll Bring

• Degree in Pharmacy and above, with valid Singapore Pharmacy Council registration
• At least 1 to 2 years of pharmacist experience, preferably in outpatient, hospital or polyclinic settings
• Experience in dispensing, medication counselling and clinical services
• Strong communication and patient-care skills
• Good teamwork and ability to work independently
• Flexibility to work across different clinic locations
• Meticulous with a strong sense of responsibility and patient safety
