The Veterans Integrated Service Network (VISN) 6 Clinical Resource Hub (CRH) Primary Care (PC) Clinical Pharmacy Program Manager, is a leadership position located in the VISN 6 CRH. The CRH Clinical Pharmacy Program Manager is under the general supervision of the Director of the CRH Primary Care. This position is located within the Clinical Resource Hub Primary Care Service at the Central Virginia VA Health Care System, Richmond, VA.

As a part of the VISN 6 CRH Team, the CRH Clinical Pharmacy Program Manager will contribute significantly to the completion of projects and utilization of resources to support the mission and goals of the VISN 6 CRH. This individual is responsible for providing CPP specific education and consultation to CRH and VISN leadership, ensuring that CRH services are completed at the highest standard. The CRH PC Clinical Pharmacy Program Manager is required to exercise a high level of leadership and organizational skill, independent judgment, problem-solving ability, and content expertise in program development, implementation and evaluation. The position requires the ability communicate effectively with field staff and VISN, CRH, and VA Central Office (VACO) leadership on a variety of clinical and administrative strategic priorities. This position is responsible for the leadership, management, operations, and oversight of CRH clinical pharmacy programs across the VISN. Duties include, but are not limited to: Provides leadership, management, and oversight of respective CRH PC CPP providers and the efficient implementation of these services throughout the VISN. Works collaboratively with VISN and Facility Pharmacy leadership to ensure VISN CRH CPP services are consistent with professional standards, gaps and needs. Collaborates as needed in the development and maintenance of written policies and procedures defining operational requirements for CRH PC and other VISN 6 CRH CPP programs. Maintains strong relationships and communication with various national (e.g., National CRH Leadership Team), VISN, and facility level leadership to ensure CRH PC CPP provider functions meet expectations and proper requirements. Uses advanced clinical knowledge and virtual expertise to promote staff involvement in planning, decision-making, and evaluating outcomes. Makes recommendations to the Director of CRH PC regarding CRH PC and CRH strategic direction, productivity, improved process performance, and capital utilization. Travels to provide training and implementation of VISN 6 CRH PC and CRH CPP provider services. Executing therapeutic plans utilizing the safest and most cost-effective medication treatments. Ordering, subsequent review, and action on appropriate laboratory tests and other diagnostic studies necessary to monitor, support, and modify the patient's medication therapy. Ordering consults and referrals, as appropriate, to maximize positive drug therapy outcomes; examples include, but are not limited to, referrals for routine care or assessment to clinical nutrition professionals, prosthetics, social workers, integrated mental health professionals, or other specialty areas. Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, pharmacokinetic impact, and therapeutic outcomes as required; information is communicated with team members and appropriate alternatives to current treatment plans are provided. Promotes and monitors compliance for established drug therapy policies. Serves as an authority on drug usage, interactions, over-dosages, and compliance. Maintains a good working knowledge of information resources, both written and electronic, as well as therapeutics and disease management.
