Pharmacist (WEST) | Healthcare | Monday to Friday office hour
Location: West
Working Hours: 5 Days (Monday – Friday), 42 hours per week
Job Responsibilities
Provide pharmaceutical care as part of a multidisciplinary healthcare team under the NUHS@Home programme.
Review patients' transfer medications and contact patients prior to transfer to assess medication supply requirements.
Conduct medication counselling through face-to-face and virtual consultations.
Perform medication reconciliation to ensure accuracy and continuity of care.
Review discharge prescriptions and verify new medication orders from physicians.
Identify and flag inappropriate medications or dosages where necessary.
Assist with medication supply duties and monitor medication preparation status.
Handle other duties as assigned based on operational requirements.
