Physical Therapist (1036)

REPORTING: The Physical Therapist reports to the Director of Rehabilitation

JOB DUTIES:

• Basic knowledge of physical medicine, physical therapy modalities, anatomy and physiology.

• Basic knowledge of physical therapy machine operation, maintenance and repairs of same.

• Provides assessment, treatment, and rehabilitative services to patients who have experienced

• injury or disability disease. Provides residents with exercises.

• Adheres to standards of physical medicine practice.

• Ability to observe and evaluate treatment effect, discusses observations with staff, Physical

• Therapy Assistant, Physician, and Director of Rehabilitation.

• Responsible for the monitoring and documentation of resident treatment and response.

• Assists with formulation of a teaching plan based upon identified learning needs and assesses effectiveness of learning, family is included in teaching as appropriate, staff and physician is apprised of conclusions.

• Meets current documentation standards and policies.

• Adheres to facility’s Policy and Procedure.
