This position offers the opportunity to build lasting relationships with patients while working alongside a collaborative interdisciplinary team dedicated to delivering exceptional rehabilitation services in skilled nursing and post-acute care.

What You’ll Do
- Evaluate patients and develop individualized plans of care
- Deliver evidence-based physical therapy focused on improving mobility, balance, strength, safety, and functional independence
- Provide rehabilitation services at Lamoni Specialty Care and Westview Care Center
- Collaborate with physicians, nursing staff, families, and interdisciplinary team members
- Supervise Physical Therapy Assistants in accordance with state licensure requirements
- Complete timely, accurate documentation supporting quality patient care and regulatory compliance
